What happens in the CONSULTING stage:

* Developing a Housing Focused Person-Centered Plan to get to know the person and their housing needs, wants, and goals.
* Helping person to make an informed choice in their housing transition or sustaining services provider.
* Offering resources related to non-housing goals.
* Coordinating with other service providers already working with the person.

PLEASE NOTE: Housing consultation is not available to people who receive Medical Assistance-funded case management including home and community-based waiver case management, targeted case management including: Adult Mental Health, Children’s Mental Health, Vulnerable Adult/Developmental Disability, Child Welfare and Relocation Service Coordination or MSHO/MSC+ care coordination.
What happens in the TRANSITIONING Stage:

Housing Transition Services helps a person plan for, find, and move into housing and may include:
* Helping the person think about preferred housing.

* Helping with the housing search, application process, and organizing the move.
​
* Supporting the person in applying for benefits and services to afford their housing, including helping the person determine which benefits they may be eligible for.
* Developing a budget.
* Understanding a lease.
What happens in the SUSTAINING Stage:

Housing Sustaining Services helps a person keep their housing after they have moved in and may include:
​
* Coaching to develop and maintain key relationships with property managers, landlord, and neighbors.
​
* Advocating with community resources to prevent eviction when housing is at risk and maintain person’s safety.
​
* Continued training on being a good tenant, lease compliance, and household management.
* Problem-solving to maintain housing stability.
​
* Supporting the building of natural housing supports and resources in the community including building supports and resources related to a person’s culture and identity.
